This is what the modified version of my flyer looks like. If you want me to send you a hi-res version of it via email, leave me a request post and I will send a .jpeg of the file to you. It lacks practice-specific data so that you can fill it in yourself with Photoshop. If you like, I can custom-fill in info which pertains to your practice, and then send you a high quality PDF file of the completed file. Less work for you, more fun for me.
