I guess neither of the "normal" criminals (with 10 or 20 killings) can hold a candle to monarchs and modern politicians.
Also any slave owning woman might be part of the list.

However, queens and presidents are always part of a system, it's hard to tell if they are to blame for the crimes or not.
For example, Queen Victoria has millions of victims, who died of the famines in Ireland and India. (Ireland was a natural disaster, but english politics did nothing to help. India was directly caused by changing from silver to gold standard money.) But in the 19th century, the Queen had much less power then in the 16th.


Anyway, how about Ilse Koch?
